The days are getting shorter, the weather is getting cooler, and the leaves are starting to change color, which means one thing– cold and flu season is right around the corner!
You can prevent getting sick by washing your hands, covering your coughs, cleaning high-trafficked surfaces AND getting your flu shot!
As part of our commitment to maintaining a healthy workplace and protecting the children and families we serve, the Children's Home Society annual flu shot requirement will go into effect starting in October. If you choose to opt out of the shot, a signed declination form must be submitted to HR before November 15.
Key Dates & Requirements:
- Campus Vaccinations: Each campus will have on–site vaccinations available.
- SF CHS Campus – October 9, from 9 AM to 12:30 PM & 1:30 to 4 PM.
- SF Shelter – October 28, 8 to 10 AM, 12 to 1:30 PM and 4 to 6 PM.
- BH-CBS (Jolly Ln) – October 9, 10:30 to 11:15 AM
- BH-CHS – October 9, 12 PM to 2 PM
- Deadline for Submission: November 15, 2024
- Verification: Proof of flu shot must be turned in to HR.
- Declination Form: If you choose not to get the flu shot, a signed declination form must be submitted to HR before the deadline.
How to Submit:
- Flu Shot Verification: Provide documentation from a healthcare provider or vaccination clinic.
- Declination Form: Forms are available through HR, Good Works, and can be submitted via email or in person.
