2023 Tax Forms are available to download in Paylocity. In the Web Application you will find these in your self-service portal > Pay> View Current Check >Tax Forms.

These forms will need to be downloaded before printing to see the entire tax form.

In the Mobile Application you will find these under Pay >Tax Forms >2023 >send to CHS or personal email by clicking the envelope in the bottom right corner.

You will need to create a password before you click send.
