CHS Community Based Services (CBS) staff recently attended the Transformational Collaborative Outcomes Management (TCOM) conference in Lexington, KY. The presentations at this conference reflect the use of TCOM and its tools. The TCOM approach is grounded in a philosophy of a single shared vision—helping people achieve their health and wellness goals as they navigate healthcare, child welfare, juvenile justice, behavioral health, education and other complex systems.

In the second photo, Chelsie and Nichole stand with Dr. April Fernando. She is helping develop a CANS 3.0 tool for service providers in South Dakota to use more efficiently. This will better help CHS programs (and other community partners) collect, analyze, and utilize assessment data to create treatment plans and engage families.
